Buy Passport Forms Online
As of July 1, 2010, all passport applicants will need to provide evidence of United States citizenship. Acceptable proof may include documents like birth certificates, Buy Passport Forms Online Consular Report of Birth Abroad/Certificate of Citizenship forms (DS-1350), naturalization papers, or previously issued full-validity United States passport books.
Secondary evidence of citizenship may be required in certain instances and can include documents that verify your legal name, parents’ names, date and place of birth, as well as official seal or court-issued document verifying your identity as proof of citizenship.
