How to Order Passport Forms Online

When ordering passport forms online, be aware that the application process can take 7 to 10 weeks. If international travel plans require your passport sooner, expedited services are available at an additional fee.

Individual applicants are responsible for being prepared with all required documents when applying. It is not the role of the Clerk’s Office to decide which documents you need for travel purposes; that responsibility lies with the Department of State.

As part of your supporting documentation, one primary item should be your United States birth certificate. Other documents listed in the Supporting Documentation Table may also be submitted as evidence of citizenship and identity. Every applicant must also submit one WHITE background color photo that meets U.S. Department of State specifications; 2 inches by 2 inches in size. It must have been taken within six months and not include eyeglasses in it.

Passport fees are collected by the Clerk’s Office as part of its processing and execution fee; however, any remaining balance must be sent directly to the U.S. Department of State by money order or cashier’s check (exact amount please). Accepted methods of payment for Department fees include Visa, MasterCard, American Express and Discover cards.
